  • Administration of Over-the-Counter Medication in Schools—Effective for the 2024–2025 School Year

     

    Dear Parents and Guardians,

    If at any time you feel that your child will need over-the-counter medications throughout the school year, as listed below, you MUST have a completed

     Medication Authorization Form and Release on file EACH school year in addition to your signature.

    Acetaminophen (Tylenol), Ibuprofen (Motrin), Benadryl, Triple Antibiotic Ointment, Hydrocortisone Cream, Cough Drops, Caladryl or Bactine 

    This form will allow for treatment of non-urgent medical needs, without disrupting your day to request permission every time these needs arise for the student.  

    The school health nurses welcome your questions and concerns.

    *Students are not allowed to bring medications to school unless all over-the-counter forms, self-medicating forms and Individualized Healthcare Plans have been returned to the school nurse for review and approval each school year.*
